According to the American Society of Plastic Surgeons (ASPS), nearly 300,000 women had breast augmentation performed in 2010. Roughly a 40 percent increase since 2000. Thanks to the many safe options available to patients, these numbers will certainly increase in the next 10 years.
Most women opt for this surgery to enlarge and enhance their breasts, but others do it to make their breasts look more symmetrical. When deciding on whether to move forward with breast augmentation, the burning question for many women remains – “How large should I go?” Dr. Samuel J. One of the most common plastic surgery procedures is also one of the oldest: breast augmentation. Since their invention 50 years ago, silicone implants have been carefully studied and refined. Now in their fifth generation, they are safer and produce better results than ever.
In 1961, Dow Corning researchers Thomas Cronin and Frank Gerow created the first implant. It was a rubber shell filled with silicone gel. They were firm to the touch and certainly did not have the kind of give and flexibility that implants have today. Another difference was how they were implanted. It was believed then that implants had to be attached to breast tissue in order to stay in place. In the early stages, a silicone mesh was fixed to the implant, but was soon discontinued. As the years went by, the shell gradually became thinner and less bulky.
It was not until the 1970s that the second generation of implants came to be. These were thinner yet and contained more liquid silicone filling. The evolution of the product was there, but this was a problematic time for the still new technology as these implants was said top rupture easily and leak silicone. This lead to many lawsuits against Dow Corning, not to mention quite a bit of controversy over the safety of the procedure. The FDA even banned the implants’ use for 14 years.
In the early 1980s, manufacturing improvements brought the implant back in improved and safer form. Third generation implants’ cover material was reinforced with silica to help against silicone leaking. The fourth generation followed soon thereafter in the mid-1980s with the focus centering on the implants’ filling. Thicker, silicone gel filing finally put implants back on the map and these fourth generation versions regained FDA approval in 2006.
Today, there is a fifth generation of implant that is better than ever. The silicone material is not only thick and cohesive, it’s formally more stable. The gel these days is a hybrid material that’s solid in shape but has enough softness to give the implants a more natural give and feel. The implants have a bounce to the touch and when cut open, they not only retain their shape, but do not leak.

Yes, breast implants are deemed safe; but that doesn’t mean they are meant to last a lifetime.
The Food and Drug Administration (FDA) recently reported in a press release that the longer a woman keeps her implants, her chances of experiencing complications increases.
It also warned that one in five women who have had breast implants, whether to increase their cup size or for reconstruction purposes, may have to undergo another operation within 10 years. “For patients who received implants for breast reconstruction, as many as one in two will require removal 10 years after implantation,” the press release stated.
Additionally, one in five patients who received implants to boost their appearance will need them removed within 10 years.
Fortunately, a patient’s risk of experiencing these problems can be reduced by undergoing surgery with a board certified plastic surgeon. As a New York City breast augmentation expert, Dr. Beran has found that most breast implant complications occur from hardening of the breast tissue around the implant or a rupturing of the implant – or as the FDA release reported, “wrinkling, asymmetry, scarring, pain and infection.”

There have always been questions regarding the safety of breast implants: Can they be harmful if they leak? Do silicone implants cause a greater health risk than saline breast implants? Can breast implants cause cancer? This last question is one that has been increasingly popular in recent years due to some related cases of anaplastic large cell lymphoma (ALCL), a very rare type of cancer, and breast implants. In a recent U.S. Food and Drug Association (FDA) press release, it is reported that there is a possible association between saline and silicone gel filled breast implants and this type of cancer. Usually found in the lymph nodes and skin, those who have been diagnosed with ALCL typically have undergone breast implant surgery.
However, these findings are still being further investigated in order to learn more regarding the source of the cancer. The FDA is asking all healthcare professionals to report any cases of diagnosis and urge patients to be aware of any changes to their breasts after surgery. There are 60 cases worldwide; however, some of these may be duplicate reports and there may still be some that have not been reported. In any case, it is important to always be cautious when considering breast enhancement surgery. Be sure to ask of the risks related to this procedure in order to be better prepared should you experience any complications.

When considering breast augmentation surgery, one of the most common questions is: Which breast implant is best? The two options available are saline and silicone gel filled breast implants. Each carries its own set of pros and cons, which are thoroughly discussed during your initial consultation. While they are both made of a thin, silicone shell, they have very different fillings.
Saline implants are placed in the breasts through one of various incision sites – under the breast, around the nipple, or through the armpit. The empty shell is placed either beneath or above the chest muscle and then filled with the desired amount of saline. Silicone gel breast implants can also be placed through any of theses incisions. These come pre-filled; therefore, the volume is changed by choosing a specific implant size. In regards to natural look and feel, silicone implants are ideal. However, should a silicone gel filled implant rupture, it will not be as noticeable as when a saline breast implant leaks. One important factor that patients must take into account is the age restriction of each implant type: 18 years old for saline and 22 years old for silicone gel.

Women who visit my New York plastic surgery practice to learn about their options in breast surgery often have the same question: Do I need a lift or augmentation? The answer to this depends on various factors – breast shape, size, nipple alignment, and overall body shape and size. If a patient has small breasts with minimal or no sagging, breast augmentation may be the best option for her. When there is excess sagging and loss of volume, a breast lift may be a better choice. However, there are some circumstances that would be ideal for a lift with augmentation.
In addition to breast enlargement and lift, patients may inquire about reduction, reconstruction, and implant revision. If you have large, pendulous breasts that causes neck, shoulder, and/or back pain, a breast reduction may alleviate these symptoms and provide a more balanced contour. Breast cancer patients who have undergone a mastectomy (breast removal) may be interested in a reconstructive procedure. Also, if you have had a primary breast enhancement procedure and were left with unsatisfactory results or experienced complications, revision surgery may be of great benefit.
